The following forms and applications are available to members and friends online:
The Deacon Board, in recognition of the responsibility to use God's property fully and appropriately, has issued the following guidelines for the use of the church's facilities and equipment by church members:
- Members of the congregation who wish to use the building or equipment must obtain permission from a member of the deacon board.
- To facilitate planning and avoid conflicts, please give as much notice as possible.
- Tables, chairs, bibles, Psalters, etc. are available to you and you are encouraged to make use of them as you practice Christian hospitality. Notification must be given and borrowed items returned promptly.
- The sanctuary and classrooms are off limits unless specifically requested. This will allow our teachers to keep their classrooms and supplies in order.
- If youth are present and/or part of the event, they must be supervised at all times. You are responsible for adhering to PA Act 153 and ensuring the appropriate clearances have been obtained, if applicable to the event.
- Custodial services may not occur between your church event and other church events. You are responsible to make sure the building is clean and ready for other use.
- Use the following checklist to make sure your clean-up is complete:
- Rooms used are cleaned and ready for use the next day.
- Restrooms:
- Commodes are flushed.
- Faucets are turned off.
- Windows closed and latched.
- Lights turned off.
- Garbage is completely enclosed in cans and lids are tight.
- All lights are off and all doors are bolted and/or locked.

- Friends of the congregation or community groups who wish to use the building or equipment must ask for and obtain permission in writing from the Deacon Board prior to using the church's facilities.
- The individual or group is responsible to leave the building in good condition and to pay for supplies used and any damage to the building or equipment.
- The individual or group will reimburse the church for custodial service or make sure the building is as clean as you found it.
- The Deacon Board may require a member of the congregation to be present during the function and be reimbursed for their time by the individual or group.
- If youth are present and/or part of the event, they must be supervised at all times. The individual or group is responsible for adhering to PA Act 153 and ensuring the appropriate clearances have been obtained, if applicable to the event.
- The use of alcohol or tobacco is not permitted on church property and the building is not to be used for political purposes.
- If the use of the piano is requested, the Deacon Board may require that it be tuned at the expense of the individual or group.

Please notify a Deacon, Elder, or Pastor of your need for assistance moving four (4) weeks before the move date. The Deacons will make the announcement to the congregation on the next Lord’s Day (minimum 3 weeks).
REMEMBER:
- Purchase moving supplies including boxes, packing material, tape, labels, etc. (6 weeks)
- Label and number boxes, and code by room (make a master list with inventory are key items)
- Begin packing items that aren't used frequently (6 weeks)
- Reserve moving rental truck (2 weeks)
- If moving major appliances, make sure they're cleaned, unplugged, freezers defrosted, etc.
- Disassemble large furniture (beds, tables, cabinets, etc.) and make ready for move the night before.
- Utilities should be turned off in your current home one day after your move-out date.
- Utilities in your new home are turned on one day before your move-in date.
- Dispose of all flammable items that can't be transported (paint, chemicals, etc.).
